I agree to complete any required induction before using the gym, follow TFP rules and staff instructions, use equipment correctly, report defects and incidents, not train while unwell or impaired by alcohol or drugs, follow emergency and unstaffed-access procedures, keep my access credentials personal and not admit anyone else.

I confirm that my application and health answers are complete and accurate. I understand that physical exercise and using gym equipment involve ordinary inherent risks, including injury, even when reasonable care is taken. I voluntarily accept those ordinary risks, will exercise within my capabilities, tell TFP if my health changes, and stop exercising and seek appropriate help if I feel unwell. I understand that this declaration forms part of my membership agreement. It does not exclude or limit TFP’s liability for death or personal injury caused by negligence, or any other legal rights or responsibilities that cannot lawfully be excluded or limited.

I explicitly consent to Top Floor Performance processing the health information I provide solely to assess safe participation and manage my membership. I understand I can withdraw this consent by contacting TFP, although TFP may then be unable to provide membership.
